Prostate cancer is the most common tumor form in men in Sweden according to cancer foundation. It mainly affects men over 60 years of age but is becoming common as early as the age of 50. Prostate cancer in the early stages usually does not cause any symptoms and is therefore difficult to detect. Do you have an increased hereditary predisposition to prostate cancer, are worried about getting prostate cancer, or do you have a known prostate disease and want to check this? MRI of the prostate examines the prostate gland, seminal vesicles, lymph nodes and skeleton and provides a very detailed image of the shape, size and consistency of the prostate gland as well as the surrounding lymph nodes and skeleton, which can clarify whether there is benign inflammation, prostate enlargement or a malignant tumor.

Why MRI scan, isn't PSA test enough?

An elevated PSA value may indicate prostate disease, which can be benign inflammation (prostatitis), prostate enlargement and urinary tract infection, or malignant prostate cancer. Therefore, the PSA value is relatively nonspecific and can lead to unnecessary interventions when it is elevated. Other tumors are aggressive, malignant, and need to be detected and treated in time.

Prostate MRI can easily and clearly show at the mm level where potentially dangerous areas are located in the prostate or in other organs in the pelvis. Based on the findings from the MRI examination, one knows exactly where to take tissue samples to investigate the suspicious changes. Otherwise, one risks taking a lot of tissue samples systematically with ultrasound solely based on a falsely elevated PSA value. Therefore, prostate MRI together with PSA testing is a much more reliable and safer method than a PSA value alone [1].

MRI is revolutionizing the detection of prostate cancer.

When you undergo a painless MRI scan of the prostate before taking a tissue sample, the doctor knows whether there are signs of a dangerous tumor or not and exactly where the suspected tumor is located. Relying solely on PSA tests and ultrasounds can lead to a lot of unnecessary tissue samples.
MRI examination of the prostate therefore provides fewer needle stick infections and a significantly more reliable diagnosis.

In a study in Jama magazine showed that 30% more high-risk tumors and 17% fewer low-risk tumors were found when the same patients were examined with MRI-guided biopsies compared to ultrasound-guided biopsies alone [2]. According to a study conducted at the Karolinska Institute [3] and published in the New England Journal of Medicine [4], MRI prostate and targeted tissue samples can halve overdiagnosis in prostate cancer screening.

What happens if suspicious changes are detected that indicate prostate cancer?

If suspicious tumor changes are detected during a prostate MRI together with an elevated PSA value, tissue samples (biopsies) of the prostate need to be taken. Tissue samples involve a needle prick into the prostate gland. Our doctors will send a referral to follow up and investigate your condition.

Benign prostatic hyperplasia and prostatitis

MRI of the prostate also shows benign enlargement of the prostate gland or Benign Prostatic Hyperplasia (BPH), which affects 50% of all men over 50 years of age and 90% of all men over 80 years of age.

Benign inflammation of the prostate gland, known as acute or chronic prostatitis, can also be detected via MRI, which occurs in up to 10-14% of all adult men [5].

What is prostate MRI?
  • Magnetic resonance imaging (MRI or MRI) is a medical technique for imaging diagnostics using a magnetic resonance imaging (MRI) camera.
  • MRI involves taking images using magnetic fields and radio waves without any X-ray radiation, which makes it completely harmless.
  • The technology is used to detect diseases, map injuries and follow up treatments at the millimeter level in almost all human organs, such as tumors, inflammation, fractures, osteoarthritis, stroke and changes in blood vessels and bones.
How does a prostate MRI work?
  • MRI examinations are performed at X-ray clinics. The images are first reviewed by a specialist in X-ray (radiologist). Prevcare's specialist then reviews the results, writes a summary and recommendations. If necessary, a referral is sent for further processing.
  • When you are examined with an MRI camera, you lie down on a bed that is moved into a so-called magnetic tunnel. There, you lie still while the images are taken. A nurse operates the camera from an adjacent room, and you can talk to the nurse at all times through a microphone and speaker.
  • After the MRI examination, you can leave the clinic immediately.
  • Prostate MRI takes about 20 minutes.
PSA test for men
  • For full-body MRI, men need to supplement with a PSA test, which is taken via blood test. Specialist doctors decide who needs to take a PSA test before the examination.
  • PSA stands for prostate-specific antigen and the test measures the amount of prostate-specific antigen in the blood. The test may indicate a tumor change, but the most common reason for the value to be elevated is benign reasons such as benign prostatic hyperplasia (prostate enlargement), urinary tract infection and acute/chronic prostatitis. By combining the PSA test with an MRI scan, the doctor gets a more reliable idea of ​​whether there are signs of cancer in the prostate gland.   • No preparation is required for a PSA blood test. When you arrive at the lab/clinic, you take a number and wait until your number is called. You only need to show your ID to the nurse before the test so she knows which blood sample is to be taken.
Who is not suitable for Prostate MRI?
  • If you have electronic equipment implanted in your body, such as a pacemaker, insulin pump or cochlear implant, for example.
  • If you have had surgery on your head or heart and have metal clips implanted instead of stitches after surgery.
  • If you have metal splinters in your eye or other part of your body.
  • If you answered yes to any of the above, tell us so we and our doctor can look at what options are available.
  • Fixed dental fillings, dental implants, prostheses, hip-knee prostheses, and screws in the arms and legs are not an obstacle to an MRI examination.
How do I prepare for a Prostate MRI?
  • Please state in your health form if you have had previous X-ray examinations. This is so that our doctors can assess your condition as accurately as possible.
  • It is important that you state if you have diabetes, kidney problems or other illnesses in your health form. This is so that our doctors can assess your condition as accurately as possible.
  • It is necessary to tell your doctor if you are pregnant. MRI is generally not recommended during the first trimester of pregnancy. In such cases, the examination may be postponed until after the baby is born.
  • It is important that you tell us if you suffer from claustrophobia, or have a general feeling of discomfort in confined spaces. In this case, a discussion can be held with the responsible doctor regarding whether it is necessary to prescribe sedatives to make the examination as comfortable as possible. If you think that you are calmed by music, there is also, in some cases, the possibility of playing music during the examination for your comfort.
  • Also take the opportunity to leave all forms of jewelry and clothing that contain metal at home before you come for the examination.
  • Otherwise, it is good to discuss with the responsible specialist to see if there is anything above that cannot be fulfilled. At Prevcare, we are usually flexible regarding different solutions.
