What is the axis?
The shoulder is a joint region where the humerus, scapula, and clavicle meet. The largest joint is the ball-and-socket joint between the humerus head and the shallow socket of the scapula. In addition, there are smaller joints and gliding surfaces that allow the scapula to move toward the rib cage.
The high mobility allows the arm to be lifted, rotated and moved far away from the body. At the same time, the shallow joint socket means that muscles and soft tissues must provide a large part of the stability.

Anatomy and function of the shoulder
The rotator cuff consists of four muscles and tendons that surround the head of the shoulder joint. They help to center the joint when the arm moves. Above the tendons is a bursa that reduces friction under the protrusion of the shoulder blade.
The joint capsule and ligaments limit the outer positions and contribute to stability. The movement of the shoulder blade is at least as important as the movement in the ball and socket joint itself. If the interaction is disrupted, the strain on the tendons and muscles can increase.
Shoulder function is affected by posture, thoracic spine, neck, and how the arm is used in work or sports. Therefore, training is often focused on the entire shoulder and axillary region.
Common causes of shoulder pain
Common complaints include tendon overload, bursa irritation, frozen shoulder, osteoarthritis, instability, and injuries after falls or forceful movement. Pain when lifting to the side can occur with several conditions and is not sufficient alone for a reliable diagnosis.
At shoulder impingement The term is used for pain that occurs when tissues become irritated in a confined space. The symptoms still need to be evaluated along with mobility and strength.
Pain radiating down the arm, numbness, or impact on hand strength can sometimes come from the neck or nerves rather than from the shoulder joint itself.
When should you seek care for your shoulder?
Seek medical attention quickly after a fall or blow if you cannot use your arm, if your shoulder appears out of alignment, or if the pain is very severe. Sudden weakness after an injury may indicate a major tendon or nerve injury.
Planned assessment is appropriate if pain does not improve, if you have clearly impaired mobility, recurrent instability, or nocturnal pain that affects sleep.
Shoulder pain accompanied by chest pressure, shortness of breath, cold sweats or general weakness should be assessed urgently as the cause may lie outside the shoulder.
Seek emergency care if there is a suspected dislocation, fracture, pronounced circulatory or nerve damage, or if shoulder pain is combined with chest pain and breathing difficulties.
How are shoulder problems investigated?
The assessment begins with how the problem arose, which movements cause pain, and whether strength or sensation is affected. The healthcare provider examines active and passive mobility, strength, tenderness, and stability.
For many long-term conditions, clinical examination and a period of adapted training are more important than immediate imaging. Regular X-rays can show bones, osteoarthritis or calcification, while ultrasound can assess certain tendons dynamically.
Which examination is chosen depends on whether the question concerns the skeleton, tendons, ligaments, cartilage, nerve damage or another disease.
When might MRI of the shoulder be relevant?
MR axis can provide detailed images of the rotator cuff tendons, muscles, ligaments, cartilage, bone marrow and other soft tissues. The examination may be relevant in cases of persistent complaints, suspected major injury or when clinical examination does not provide sufficient answers.
MRI is not always the first step. For some injuries, X-rays are needed first, and for many overuse injuries, treatment can be initiated based on symptoms and examination findings.
Imaging findings need to be linked to the symptoms. Changes can also be present in people without pain, and a normal MRI response does not rule out all possible causes.
Stress, rehabilitation and return to activity
For many shoulder conditions, it is wise to reduce the movements that clearly provoke the pain without keeping the arm completely still. Complete relief for a long time can increase stiffness and muscle weakness. A physiotherapist can help adjust mobility, strength and load according to which tissue appears to be affected.
Rehabilitation often focuses on controlling the scapula, rotator cuff strength, and gradual return to work or sports. Improvement is common over weeks or months rather than days. Pain does not need to be completely gone before activity can be resumed, but the load should be manageable and not cause significant, lasting impairment.
After a dislocation, major tendon rupture or fracture, the plan looks different and may include immobilization, surgery or more structured rehabilitation. Age, activity level and previous injuries affect the decision. Imaging is then part of the whole and not a stand-alone answer.
If an MRI scan shows changes, the next question is whether they match the symptoms. Tendon changes and minor cartilage changes can occur without being the main cause of the pain. Clinical examination and function therefore always need to be considered before choosing a treatment.
