What is a joint?
A joint is a connection between two or more bones. Some joints, such as the shoulder and hip, allow for large movements. Others, such as the joints between certain vertebrae, have less movement but contribute to stability.
In a synovial joint, the ends of the bones are covered by cartilage and surrounded by a joint capsule. The synovium produces synovial fluid that lubricates the joint. Ligaments, muscles, and tendons help control and stabilize movement.
Symptoms can be described as joint pain , stiffness, swelling or instability. The symptom itself does not indicate which structure is affected.
How do the joints work?
Cartilage distributes load and provides a smooth gliding surface. It lacks its own blood vessels and receives nourishment through alternating between loading and unloading.
Muscles and tendons produce movement, while ligaments and joint capsules limit movements that can damage the joint. The nervous system registers the position of the joint and helps the body react quickly.
Regular movement and appropriate load are important for joint health. Too little activity can cause weakness and stiffness, while rapid or one-sided load increases can irritate tissues.
Common causes of joint pain and stiffness
Osteoarthritis of the knee and osteoarthritis of the hip often cause pain and stiffness when moving. Inflammatory joint disease can instead cause significant swelling and prolonged morning stiffness.
Acute pain in one joint can be caused by injury, gout, infection, or bleeding. Pain in multiple joints can also occur due to infections, autoimmune diseases, hormonal conditions, or drug side effects.
Pain near a joint does not always come from the joint itself. Tendon attachments, bursae, muscles and nerves can cause similar problems.
When should joint problems be assessed by healthcare professionals?
Seek medical attention quickly if a joint becomes red, warm, severely swollen, and very painful, especially if you also have a fever or feel unwell. A joint infection needs to be ruled out urgently.
After trauma, you should seek care if the joint is misaligned, unstable, cannot be loaded, or if strength and sensation are affected.
Planned assessment is appropriate for recurrent swelling, prolonged morning stiffness, problems in multiple joints, or when the pain limits work, sleep, or daily functioning.
A suddenly red, warm and very painful joint with fever or general weakness should be assessed on the same day.
How are joint problems investigated?
The evaluation begins with a symptom pattern, medical history, and physical examination. The healthcare provider assesses swelling, warmth, range of motion, stability, and whether the pain is coming from the joint or surrounding tissue.
Blood tests may be relevant if inflammation, infection, or other systemic disease is suspected. Sometimes synovial fluid needs to be analyzed to detect crystals, infection, or bleeding.
Regular X-rays show skeletal and osteoarthritis changes. Ultrasound can show synovial fluid and superficial tendons, while MRI can provide details of cartilage, bone marrow, ligaments and deeper soft tissues.
Which imaging diagnostics are suitable for joints?
There is no single test that is best for all joints and all issues. X-rays are often the first step in trauma and osteoarthritis. Ultrasound can be used for fluid, inflammation, and certain tendons.
MRI may be relevant when the question concerns the meniscus, ligaments, cartilage, bone marrow or deeper soft tissues. Read about targeted examinations such as MRI knee , MRI hip and MRI shoulder.
Imaging findings need to be linked to symptoms and function. Many changes are common with increasing age and do not necessarily cause pain.
How movement, load and recovery affect joints
Joints generally benefit from regular movement because loading helps cartilage and surrounding tissues to metabolize nutrients and maintain function. However, the loading needs to be adjusted to the capacity of the tissue. A rapid increase in exercise or one-sided activity can cause pain even without damage to the joint.
In osteoarthritis, physical activity and strength training are often central parts of treatment. Pain during or after activity does not automatically mean that the joint is damaged, but the level needs to be manageable. The goal is to improve function and endurance rather than completely avoid strain.
Inflammatory joint diseases require a different strategy. In case of active inflammation, drug treatment and rheumatological follow-up may be needed to protect the joint. Exercise is still valuable but should be adjusted according to disease activity and general condition.
An imaging finding should not be assessed in isolation. Osteoarthritis, meniscal changes, and other age-related findings are common even in people with few symptoms. Treatment decisions should therefore be based on pain patterns, function, clinical examination, and the patient's goals.
